What Is Form CRS?

Form CRS is the relationship summary that certain SEC-registered broker-dealers and investment advisers give retail investors. It is short on purpose. It covers what services the firm offers, what you pay, how the firm and its professionals make money and where their interests can diverge from yours, the standard of conduct that applies, and whether the firm or its professionals have reportable legal or disciplinary history.

Because every covered firm answers the same questions in a similar order, the form is one of the few documents that makes two financial firms comparable. The Form CRS guide explains each section and how to read it. This worksheet is the place to write down what you find.

Form CRS Comparison Worksheet

Type what each firm's documents say, in your own words. Leave a box empty if you have not found the answer yet. Do not enter your name, account numbers, or other personal information: the worksheet is about the firms, not about you.

Exports contain only what you typed. They are created in your browser and never uploaded.

Saving

Not saving. Notes disappear when you leave or reload this page. Tick the box to keep them in this browser only.

This worksheet organizes disclosures you copy from primary documents. It does not rate, rank, endorse, or select a financial professional, and it is not investment, legal, or tax advice.

How to Use the Worksheet

  1. Gather one document set per firm. Open each firm's Form CRS, then its Form ADV (advisers) and its BrokerCheck or IAPD record.
  2. Fill in one column per firm. Start with the legal firm name, because the entity that holds your account may differ from the brand name on the website.
  3. Copy facts, not impressions. Write what the document states, for example the fee schedule as printed. Use the "Look in" hint under each question to find it.
  4. Paste the source links. Only web addresses that start with https:// or http:// are accepted, so each note stays traceable to its document.
  5. Watch the blanks. A blank means you have not found the answer or the document did not give one. Both are worth a question to the firm.
  6. Print or export. Keep a copy of your notes, or bring them to a conversation with the firm.

Where Each Source Document Is Found

The Form CRS guide shows how the pieces fit, and how to verify a financial professional walks through checking the full record.

What This Worksheet Does Not Do

  • It does not produce a winner, a score, a ranking, or a recommendation. Firms with different service models cannot be collapsed into one number without hiding what matters.
  • It does not look anything up. No firm data is fetched, and no live feed is connected. Everything in the table is what you typed.
  • It does not decide whether a firm suits you. That depends on your own situation and is outside what a worksheet can judge.
  • It does not verify your notes. Check them against the primary documents linked above.
  • It does not collect personal information, and it has no fields for it.

The blank-field count is structural only: it counts empty boxes. A reportable disciplinary disclosure is a reason to read the underlying record, and no disclosure is not a reason to stop checking.

Frequently Asked Questions

What is Form CRS?

Form CRS is the short relationship summary that certain SEC-registered broker-dealers and investment advisers must give retail investors. It describes the firm's services, fees and costs, conflicts of interest, standard of conduct, and reportable legal or disciplinary history, and points to where more information lives. It is a summary, so it opens questions rather than closing them.

Does the worksheet score or pick a firm?

No. The worksheet never produces a winner, a score, a ranking, or a recommendation. It lines up what each firm's own documents say, one question per row, so you can read them side by side. The only number it shows is how many fields you have left blank, which describes the worksheet and says nothing about a firm.

Is what I type saved or sent anywhere?

Nothing you type is sent to Swoopr Investment or anyone else. By default the notes exist only on the open page and disappear when you reload. If you tick Remember on this device, the notes are stored in your browser's local storage on this device only, and the Clear saved notes button removes them. The worksheet has no fields for names, account numbers, or other personal information.

Where do I find a firm's Form CRS and Form ADV?

A firm's Form CRS is usually on its website, and Investor.gov lists relationship summaries. For an investment adviser, the Investment Adviser Public Disclosure site holds its Form ADV and registration record. For a broker or brokerage firm, FINRA BrokerCheck holds registration and disclosure records. Paste the address of each page you used into the source link boxes so your notes stay traceable.

Why does the worksheet count blank fields?

The count shows how much of the table you have not filled in yet, which helps you see where a firm's documents were silent or where you have not looked. A firm with more blank fields is not a weaker firm and one with fewer is not a stronger one. It is a progress marker for your own note-taking, not an assessment.
